Yes or no with a reason
A switch whose "no" needs a reason before the form can be sent.

Rules
- The reason field appears under the switch, never in a pop-up.
- Turning the switch back to yes hides the reason but keeps the text.
- Rejecting, stopping and cancelling follow the same rule with Confirm.
